A vaccination clinic will be conducted in the middle school's nurse's office on September 12th from 9 a.m. - 11 a.m. Please see attached forms that we will need filled out for Tdap & MCV4. The parents do not have to be present as long as the consent forms are signed. The price is $10 for the first shot, and $5 for each additional vaccine(Cash Only). A check is acceptable if the school is the issuing party.
